Once your form is submitted, I will reach out to you within 3 business days if I have any questions or concerns with your form. My schedule is usually booked 1-3 months out, and my updated availability is listed on my Instagram highlight "APPTS".

If you are a returning client, you do NOT need to submit a new client form. This is only for clients who have never had a service done by me at Studio E Brow Artistry. To book an annual touch up, please text, email or DM me a current picture of your brows in natural lighting and no makeup on them so I can see if they are faded enough and ready for a touch up. Typically, they need to be at least 50% faded before needing to have a refresh!

There is a $100 non-refundable and non-transferable booking fee that must be sent at the time of booking ANY appointment and goes towards the cost of your appointment. This can be paid with Venmo, Zelle or cash. Detailed instructions will be given to you before sending any payment. If you aren’t able to use one of those forms, you can pay the booking fee with a card through my booking system, but there will be a 3.5% service fee added.

Once I receive your booking fee, I will give you a link to my online booking system so that you can secure your appointment, input your personal info and put a card on file (nothing is charged when booking, it is only on file for cancellation/no-show protection).

The balance is due is payable with Venmo, Zelle or cash. If you would like to use your card on file, there will be a 3.5% service fee added to the total.
